Chronic diseases are the most common cause death in Mexico and the disease burden is rising. At the same time, the high rate of out-of-pocket payments for healthcare (45 percent of all healthcare spending) demonstrates a demand for affordable and accessible care. Salud Cercana's digital platform allows patients, primary those with cardiovascular diseases, to access a primary care network of doctors that are certified and use Salud Cercana's e-prescriptions software, as well as a referral network of specialists. Through the platform, patients receive care coordination, nutritional and psychological support, as well as notifications and reminders, to achieve their best health.
Beginning with a pilot in 2015, Salud Cercana treated more than 2,000 walk-in patients and signed up eighty patients to the full program by September 2016. The organization is currently opening two new clinics, and launched a corporate and microfinance patient management service.
